What Is Rhinoplasty?

Rhinoplasty, also known as a “nose job,” is a plastic surgery procedure that reshapes or resizes the nose. It involves making incisions to access the bones and cartilage that support the nose and then sculpting or removing tissue to achieve the desired appearance. Such refinement can be done in any part of the nose to remove a hump, refine the tip, narrow the nostrils, or change the angle between the nose and the upper lip. It is also performed to correct functional deformities that affect breathing, such as a deviated septum.

On average, rhinoplasty takes 1-2 hours and requires general anesthesia. The surgical results are permanent, although the nose can continue to undergo natural changes with age. Overall, rhinoplasty aims to create facial harmony by altering the size, shape, or proportions of the nose.
Dr. Sabry uses the “open” approach, where scars are placed inside the nostrils and under the columella (a small crease at the base of the nose). The skin is lifted, and the cartilage is sculpted to augment, reduce, or re-define the nose. Occasionally, a tip graft may be performed for better contouring. Osteotomy (bone fracture) can also be done to create a more harmonious/ balanced shape. Finally, the skin is re-draped and stitched closed. A splint is placed on the outside and inside to provide support if the septum is corrected.

Types of Rhinoplasty

Cosmetic Rhinoplasty

Cosmetic rhinoplasty is a type of plastic surgery that aims to change the shape or appearance of the nose for aesthetic purposes. It is one of the most popular cosmetic procedures. People usually get rhinoplasty due to dissatisfaction with their nose size, shape, or angle, which affects their confidence and self-esteem. During the procedure, plastic surgeons sculpt the bone and cartilage to achieve the desired look.

Reconstructive surgery is performed to restore structure and function to the nose after trauma, illness, or injury. The goal is to repair defects and deformities to improve breathing and return the nose’s natural appearance. Depending on the extent of reconstruction needed, such surgery may require multiple complex procedures, such as osteotomy and cartilage grafting. Typical candidates include patients with nasal defects from dog bites, accidents, cancer, infections, and congenital disabilities.
Revision rhinoplasty corrects or modifies the results of a previous surgery. Sometimes, functional or cosmetic problems persist after the initial procedure, making people self-conscious and dissatisfied. Common reasons for revision rhinoplasty include persistent breathing difficulties, uneven shape, unexpected scarring, contour irregularities, or an undesirable aesthetic outcome. This surgery is more complicated due to the presence of scar tissue from the previous operation and requires a highly skilled surgeon like Dr. Sabry to achieve the desired results.

Consultation & Preparation for Rhinoplasty in NYC Banner

Consultation & Preparation for Rhinoplasty

Dr. Sabry examines the patient’s nose and overall facial structure during the rhinoplasty consultation. He listens to the patient’s concerns, reviews their expectations, and explains the best options for achieving their goals. Photographs of the nose are taken from various angles to allow for a thorough evaluation of the existing anatomy and comparison after surgery. Dr. Sabry explains the rhinoplasty procedure in detail, including the incisions, techniques, and materials that will be used. He also discusses the risks and limitations of this operation.
If the patient decides to undergo rhinoplasty surgery, preoperative instructions are provided, including guidelines on eating, drinking, smoking, and medications to avoid. Arrangements are also made for the surgery date, estimated downtime needed for recovery, and details regarding anesthesia. Clear communication and setting appropriate expectations are vital parts of preparation. By following all instructions, rhinoplasty patients can maximize the results of their surgery and ensure a smooth recovery.

Rhinoplasty Procedure

General Anesthesia

Rhinoplasty is performed under general anesthesia to ensure that the patient is comfortable and immobile during this delicate procedure. Our experienced anesthesiologist monitors all vital signs and ensures maximum safety.
Incisions are made inside the nostrils and at the base of the nose between the nostrils to access the underlying cartilage and bone. The skin is lifted to expose the framework.
The main goal of rhinoplasty is to remove or rearrange cartilage to achieve the desired shape. Dr. Sabry sculpts different areas, such as the bridge, tip, nostrils, or septum, using advanced surgical techniques – trimming, scoring, or grafting.
Osteotomy involves breaking or removing nasal bone to narrow the bridge or straighten a deviated nose using a chisel or a piezo ultrasonic device.
Cartilage enhancements can better define, support, or lift the nasal tip. Grafts from the nose, ear, or rib area may be used to improve projection, rotation, and contour.
Once the structure is modified, the nasal skin is laid back over the new framework. Excess tissue is trimmed as needed.
The incisions inside the nose and at the base between the nostrils are carefully sutured for proper healing. Our facial plastic surgeon ensures perfect wound closure in this area.
After sealing the incisions, an external and/or internal splint is applied over the nose. This device helps maintain the new nasal contours as the skin heals over the freshly modified cartilage and bone framework. The splint stabilizes the nose for 1-2 weeks following the operation.

Recovery After
Rhinoplasty Surgery

The recovery period varies, but there is usually some swelling and bruising around the nose and eyes that peaks 3 days after surgery. This discomfort gradually reduces over the next couple of weeks. Pain is generally mild and managed with medication.
Patients are advised to take at least 7-10 days off work and avoid strenuous activity or heavy lifting for several weeks. Sleeping with the head elevated on a few pillows is recommended to minimize swelling. Cold compresses can also help to reduce discomfort in the first few days. The cast and splints are removed after one week, but the nose will remain swollen while the final shape is established.
Most people can return to normal daily activities after 2 weeks, although it may take up to a year for the nose to fully settle. Patients are advised to avoid any trauma or pressure to the nose during the recovery period. Follow-up appointments are crucial because they allow Dr. Sabry to monitor healing.

Candidates for Rhinoplasty

When considering a nose job, it is important to determine if a patient is a good candidate for functional or cosmetic plastic surgery. In general, rhinoplasty may benefit people of all ages, including:

Teenagers

This surgery can be performed as early as 15 or 16 in girls and 16 or 17 in boys when nose formation is almost complete. This allows us to make proper corrections before adulthood and ensure that the shape will not be affected.
Many young people choose to get rhinoplasty in their 20s or 30s if they are unhappy with their facial features or nasal breathing. This group makes up a significant portion of our patients.
Middle-aged adults may consider rhinoplasty as part of an overall facial rejuvenation plan or to correct problems that have worsened with age. Surgery might also be needed due to trauma and illness that affected nasal structures.

While less common, seniors can be candidates for rhinoplasty, performed very carefully on a case-by-case basis. This surgery may improve their quality of life, so it is often requested by our older patients.
